It is very simple and works well. I guess this has to do with Docs not being a web app and so not knowing the standard way how a field is entered. In a normal web browser, a field is typically added by hovering over the field while typing, like this. The field you'll likely be using here is the User ID in this form You should see your document and all your fields filled out. You can save this version to keep track of the data entered as you add more data to the form. I use this to keep track of how many emails I've sent out and how many emails I've received. I then create a .pdf extension and save this as .pdf (or other file extension) so I can open and edit the file directly from the .docx extension. Now if you want to export a .pdf.
